Variant From India Spreading Quickly Around World

 "The highly transmissible Covid-19 variant that first emerged in India is rapidly spreading around the world, health authorities say, intensifying the race to increase global vaccinations.

The B.1.617.2 variant, now dubbed the Delta variant, is in at least 60 countries, including the U.S. and the U.K., and British scientists recently estimated that it might be 40% to 50% more transmissible than the B.1.1.7 variant, or Alpha, which in turn is more transmissible than the original virus and quickly spread across the globe.

"Given that level of transmissibility, I would anticipate that it would actually spread around the world," Sharon Peacock, executive director and chair of the Covid-19 Genomics UK Consortium, said at The Wall Street Journal's Tech Health Event on Wednesday.

In the U.K., the Delta variant is rapidly displacing the Alpha variant, and health officials believe it is contributing to an uptick in Covid-19 cases and hospitalizations in the country, though it is starting from a low baseline. Previously, around 98% of cases in the U.K. were due to the Alpha variant, but the Delta variant has started to take over after being introduced into the country in March and now constitutes about 75% of cases, Dr. Peacock said. Covid-19 Genomics UK is a consortium of public-health and academic organizations that collects, sequences and analyzes genomes from Covid-19 tests in the U.K. to guide pandemic response efforts.

"We're seeing a major shift in the cause of infection in the U.K., and that change has happened relatively rapidly over a period of a few weeks, and it doesn't look as if that's going to really stop," said Dr. Peacock, who is also a professor of public health and microbiology at the University of Cambridge.

The variant is also becoming dominant in the Democratic Republic of the Congo, John Nkengasong, director of the Africa Centers for Disease Control and Prevention, said at the WSJ conference. Dr. Nkengasong said the risk from the variant is significant and the continued emergence of new variants speaks to the need to strengthen genomic sequencing across the globe.

Anthony Fauci, the U.S.'s top infectious-disease physician, said Tuesday that the Delta variant currently accounts for more than 6% of sequenced Covid-19 cases in the U.S. The Alpha variant is still dominant in the country.

"We cannot let that happen in the United States," Dr. Fauci said of the situation in the U.K., during a White House media briefing. He urged people to get vaccinated if they haven't been already and to make sure that they get their second dose.

The variant might also be associated with increased risk for severe disease and hospitalization, health authorities say, but that connection is less certain.

The currently available vaccines work against the Delta variant, early data suggest, but they seem to be somewhat less effective, especially after just one dose. According to data from Public Health England, both the AstraZeneca and Pfizer-BioNTech vaccines were about 33% effective against symptomatic Covid-19 caused by the Delta variant after one dose, compared with being about 50% effective against the Alpha variant.

After two doses, however, the Pfizer vaccine efficacy against the Delta variant increased to 88%, and the AstraZeneca vaccine was about 60% effective.

Both Drs. Peacock and Nkengasong expressed concern that, if the virus is allowed to continue to spread, new variants could emerge in the next few months that the current vaccines may be even less effective against.

"I think that that's one step in a series of incremental steps, or potentially there could be one large step, towards a variant that is more resistant to vaccination," Dr. Peacock said. "The way that we get on top of the emergence of variants is to stop transmission and vaccinate the world." [1]

 As the effectiveness of vaccines declines, we need to take advantage of the fact that we are vaccinated and to prepare for a potentially more severe and dangerous wave of the disease. Now is the time to go to doctors and dentists for those who need it. It is worth buying good masks. It is worth preparing for cases of fierce quarantine.

Korėjos karo metu priimtas Gynybos gamybos įstatymas, kuris paskatino vidaus pramonę paremti kariuomenę, dabar bus naudojamas, kad sudaryti viešojo ir privataus sektoriaus konsorciumą būtinų vaistų gamybai. Sveikatos ir žmogiškųjų paslaugų departamentas iš pradžių šiam tikslui išleisti 60 mln. 

Pasinaudojus Energetikos departamento paskolos teise investuoti į pažangių transporto priemonių akumuliatorių elementų gamybą ir įsteigti naujas gamybos įmones. 

 Įsteigti „prekybos smogiamąsias pajėgas“, kurios siūlys vykdymo veiksmus, susijusius su, administracijos nuomone, nesąžininga užsienio prekybos praktika, pavyzdžiui, Kinijos vyriausybės subsidijomis, kurios suardė kritines tiekimo grandines, ir sustiprins bendradarbiavimą su sąjungininkais. 

Siūloma JAV eksporto-importo banko finansavimo programa JAV gamybos įrenginiams ir infrastruktūros projektams. 

Viena prekybos priemonė reikalauja, kad Prekybos departamentas pradėtų tyrimą, dėl kurio gali būti nustatyti tarifai neodimio magnetams, kurie naudojami varikliuose ir gynybos įrangoje ir daugiausia importuojami iš Kinijos. 

Administracija taip pat siekia išspręsti puslaidininkių trūkumą, dėl kurio sulėtėjo gamyba, pradedant prietaisais ir baigiant kompiuteriais, o automobilių gamintojai labiausiai jaučia šį trūkumą. „Ford Motor Co.“, „General Motors Co.“ ir „Jell“ bei „Chrysler“ gamintojai „Stellantis NV“ visi reikalavo vyriausybės pagalbos, nors trumpalaikiai vyriausybės sprendimai buvo sunkiai suvokiami. Lustai naudojami transporto priemonių stabdžių sistemose, jutikliniuose ekranuose ir daugelyje kitų komponentų. Tačiau jie taip pat yra labai paklausūs plataus vartojimo elektronikos ir kitų gaminių, kurie išpopuliarėjo žmonių tarpe, kurie pandemijos metu buvo priversti likti namuose ". [1]  

Konsorciumą būtinų vaistų gamybai vertėtų sudaryti ir Lietuvoje. Bet Lietuvos elitas dabar užimtas kanapių legalizacijos problema, pagalvoti apie vaistus mums nėra kada.

"The review calls for a number of actions that represent a more active government role in supply-chain issues and domestic manufacturing and much of it is geared toward competing with China. They include:

Using the Defense Production Act, enacted during the Korean War to spur domestic industry to shore up the military, to form a public-private consortium for essential-medicine production. The Department of Health and Human Services will initially spend $60 million toward that goal.

Tapping the Energy Department's loan authority to invest in production of advanced vehicle battery cells and to establish new manufacturing plants.

Establishing a "trade strike force" that will propose enforcement actions against what the administration says are unfair foreign-trade practices, such as government subsidies, that have eroded critical supply chains and shore up cooperation with allies.

Proposing a financing program under the U.S. Export-Import Bank for U.S. manufacturing facilities and infrastructure projects.

One trade measure calls for the Commerce Department to begin an investigation that could lead to tariffs on neodymium magnets, which are used in motors and defense equipment and are largely imported from China.

The administration is also seeking to address the semiconductor shortage, which has caused production slowdowns, from appliances to computers, with auto makers being the most prominent to feel the squeeze. Ford Motor Co., General Motors Co. and Stellantis NV, maker of Jeep and Chrysler, have all pressed the government for help, though short-term government solutions have been elusive.

Chips are used in vehicle braking systems, touch screens and numerous other components. But they are also in high demand for consumer electronics and other products that became popular with people forced to stay home during the pandemic." [1]

A consortium for the production of essential medicines should be formed in Lithuania as well. But the Lithuanian elite is now busy with the problem of cannabis legalization, we have no time to think about medicines.

  "The Internet is roaring over a photo of Minister of Economy and Innovation Aušrinė Armonaitė. The politician raised her hand several times at the Freedom Party convention on Saturday when she invited her elected deputies to the stage. A raised hand reminded some politicians of saluting with the Nazi gesture. "I'm trying to find 10 differences between the "shit piece" and her ...", Aurelijus Veryga, a former health minister, member of the Seimas,  from Peasant party, wrote on Facebook on Monday evening. He remembered the case when A. Armonaitė, a member of the Seimas National Security and Defense Committee, called "Peasant" Audrius Šima "a piece of shit" last autumn for a similar gesture.  

September 15 the ethics guards decided that A. Armonaitė, by using the words “piece of shit”, did not violate the Code of Conduct of State Politicians. "When it comes to saluting Nazi signs in our parliament, in its heart, it provokes. I am surprised that I am under discussion, not Mr Audrius Šima. Of course, the decision of the commission was a bit surprising, I was expecting the worst, because I see that many members of the Seimas do not see a problem in this Nazi greeting of a member of the Seimas. And the third thing - however, the substance I talked about turns into fertilizer, fertilizes Lithuanian lands ... ", - then A. Armonaitė considered, having received a favorable decision of the ethics guardians." 

 Armonaitė does not turn into fertilizers, does not fertilize Lithuanian lands. She is toxic to everything that lives in Lithuania. Stinks, like a piece of shit, looks like a piece of shit, and is actually poison. 

 


Kuo pati tepsi, tuo ir susitepsi



"Internetas ūžia dėl nuotraukos, kurioje užfiksuota ekonomikos ir inovacijų ministrė Aušrinė Armonaitė. Politikė šeštadienį Laisvės partijos suvažiavime į viršų kelis kartus pakėlė ranką, kai į sceną kvietė išrinktus jos pavaduotojus. Iškelta ranka kai kuriems politikams priminė saliutavimą nacių gestu. „Bandau rasti 10 skirtumų tarp išvadintojo „šūdo gabalu“ ir išvadinusiojo...“, – feisbuke pirmadienio vakarą rašė buvęs sveikatos apsaugos ministras, Seimo narys, „valstietis“ Aurelijus Veryga. Jis prisiminė ir atvejį, kai Seimo Nacionalinio saugumo ir gynybos komiteto narį „valstietį“ Audrį Šimą praėjusių metų rudenį A. Armonaitė išvadino „šūdo gabalu“.
 
Rugsėjo 15 d. etikos sargai nutarė, kad A. Armonaitė, pavartojusi žodžius „šūdo gabalas“, nepažeidė Valstybės politikų elgesio kodekso. „Kada mūsų parlamente, jo širdyje, yra saliutuojama nacistiniais ženklais, tai ir išsiprovokuoja. Esu nustebinta, kad esu svarstoma aš, o ne A. Šimas, dėl kurio tyrimas dar vyksta. Aišku, komisijos sprendimas šiek tiek nustebino, tikėjausi paties blogiausio, nes matau, kad daugybė Seimo narių kaip ir nemato problemos šiame Seimo nario nacistiniame pasisveikinime. Ir trečias dalykas – vis dėlto ta substancija, apie kurią kalbėjau, virsta trąšomis, tręšia Lietuvos žemes...“, – tuomet svarstė A. Armonaitė, sulaukusi palankaus etikos sargų sprendimo."

Armonaitė nevirsta trąšomis, netręšia Lietuvos žemes. Ji yra nuodinga viskam, kas gyva Lietuvoje. Smirda, kaip šūdo gabalas, atrodo, kaip šūdo gabalas, o realiai yra nuodas.






Alzheimerio ligos gydymo proveržis, pagaliau

 "JAV Maisto ir vaistų administracija pirmadienį suteikė vilties milijonams amerikiečių, kenčiančių nuo Alzheimerio ligos, patvirtindama Biogeno adukanumabą - pirmąjį gydymą, kuris sulėtino kognityvinį nuosmukį. 

Plojimai laikinai einančiai pareigas komisarei Janet Woodcock už tai, kad priešinosi visuomenės sveikatos kairiųjų, kurie agitavo prieš patvirtinimą, spaudimui.

 Adukanumabo sunkus kelias link patvirtinimo rodo vaistų kūrimo iššūkius. Klinikinių tyrimų metu per kelis dešimtmečius nepavyko šimtai eksperimentinių Alzheimerio gydymo būdų. Nors kai kurie patvirtinti vaistai gali laikinai sušvelninti elgesio ir kognityvinius simptomus, nė vienas iš jų iki adukanumabo neturėjo įtakos ligos progresavimui. 

Viena iš priežasčių, kodėl proveržis atrodė esąs nepagaunamas, yra ta, kad mokslininkai tiksliai nesuvokė, kas sukelia Alzheimerio ligą. Jo skiriamasis požymis yra amiloido apnašų ir tau sankaupų susidarymas smegenyse, kuris paprastai prasideda ilgą laiką iki tol, kol žmonėms pasireiškia simptomai. Kai kurie mokslininkai mano, kad amiloido pašalinimas gali sulėtinti ligos progresavimą

 Čia pasirodo adukanumabas, kuris yra monokloninis antikūnas, klonuotas iš vyresnio amžiaus žmonių, kurių kognityvinis nuosmukis buvo neįprastai lėtas. Jis veikia, išvalant amiloidą. 

Didelė vaisto dozė vėlyvame etape pašalino 71% amiloido kaupimosi po 18 mėnesių. 

Dar daugiau nei dvi dešimtys eksperimentinių vaistų, skirtų amiloidui, neparodė naudos. Prieš dvejus metus adukanumabas atrodė dar viena nesėkmė, kai dviejų 3 fazės klinikinių tyrimų, kuriuose dalyvavo maždaug 3 200 dalyvių, vidurio taško „beprasmiškumo analizė“, akivaizdžiai neparodė jokios naudos. Biogenas sustabdė bandymus, ir tik po kelių mėnesių atlikdamas post-hoc analizę sužinojo, kad nutraukimas buvo ankstyvas. Vieno tyrimo metu pacientai, vartojantys didelę vaisto dozę, parodė reikšmingą naudą. Jie parodė, kad jų atmintis sumažėjo 28% mažiau nei placebo grupėje, o nauda buvo didesnė, atliekant kasdienes užduotis, pavyzdžiui, valgant. Pacientai, kurie kito tyrimo metu vartojo didelę dozę, reikšmingos naudos neparodė. Biogenas mano, kad taip yra todėl, kad jie gavo vaistą mažiau laiko. 

Po to, kai analizavo duomenis su „Biogen“, FDA rezultatus apibūdino kaip „išskirtinai įtikinančius“. Išorinė patarėjų grupė nesutiko, tačiau nė vienas iš jos narių aktyviai negydė Alzheimerio liga sergančių pacientų. Ekspertų grupė surašė kritiką „Biogen“ už duomenų masažą. Kairiosios grupės užsipuolė FDA už bendradarbiavimą su „Biogen“. Jie tvirtino, kad FDA sumažintų savo standartus ir pakenktų objektyvumui, jei ji patvirtintų vaistą. Tačiau dėl vaistų gamintojų ir FDA bendradarbiavimo amerikiečiai turi tris veiksmingas Covid-19 vakcinas. 

Kalbant apie skundus dėl nepakankamų įrodymų, kongresas 1997 m. iš dalies pakeitė federalinį įstatymą, leidžiantį FDA patvirtinti naujus vaistus, remiantis vienu tyrimu, kad paspartintų sunkiai gydomų ligų proveržius. FDA tinkamai pasinaudojo savo nuožiūra, o Biogenas planuoja atlikti papildomus tyrimus po vaistų patvirtinimo. Kritikai taip pat teigė, kad adukanumabo nauda yra nedidelė, tačiau tai jie tegul pasakoja pacientams ir jų globėjams. 

Auganti mokslo pažanga dažnai skatina vaistų kūrimą. Adukanumabas nepadės visiems pacientams, tačiau jis gali atidėti ligos progresavimą, kol tęsiami potencialiai efektyvesnio gydymo tyrimai. Daugiau nei 70 yra klinikiniame etape. Panašu, kad tikrasis kairiųjų prieštaravimas yra prieš didelę kainą, daugiau, nei dėl efektyvumo. 

Nauji kraujo tyrimai gali nustatyti ligą nepraėjus keleriems metams iki kognityvinio nuosmukio požymių, todėl potencialus pacientų skaičius netrukus gali išsiplėsti. Apskaičiuota, kad Alzheimerio liga JAV kainuoja 277 mlrd. dolerių per metus, o ankstyvas gydymas bus labai svarbus sulėtinant progresavimą. FDA pritarimas suteikia labai reikalingą patikinimą JAV vaistų gamintojams, kad FDA nepasiduos politiniam spaudimui ir neleis biotechnologijų proveržiams nueiti veltui“. [1]

An Alzheimer's Breakthrough, At Last


"The Food and Drug Administration gave hope to millions of Americans suffering from Alzheimer's disease on Monday by approving Biogen's aducanumab, the first treatment shown to slow cognitive decline. Credit to Acting Commissioner Janet Woodcock for resisting pressure from the public-health left who campaigned against the drug.

Aducanumab's twisted path to approval illustrates the challenges of drug development. Hundreds of experimental Alzheimer's treatments have failed in clinical trials over decades. While some approved drugs can temporarily mitigate behavioral and cognitive symptoms, none before aducanumab had shown an impact on disease progression.

One reason a breakthrough has proven elusive is that researchers haven't figured out exactly what causes Alzheimer's. Its hallmark is an accretion of amyloid plaque and tau tangles in the brain, which usually begins long before people show symptoms. Some scientists believe that removing amyloid could slow disease progression.

Enter aducanumab, which is a monoclonal antibody cloned from older people who showed unusually slow cognitive decline. It works by clearing amyloid. A high dose of the drug in a late-stage trial removed 71% of the amyloid buildup after 18 months.

Yet more than two dozen experimental drugs targeting amyloid have failed to show benefits. Two years ago, aducanumab looked to be another failure after a midpoint "futility analysis" of two Phase 3 clinical trials with some 3,200 participants apparently showed no benefit.

Biogen halted the trials, only to discover months later in a post-hoc analysis that the termination was premature. In one trial, patients receiving a high dose of the drug demonstrated significant benefits. They showed 28% less memory decline than the placebo group, and benefits were greater for everyday tasks like making meals.

Patients receiving the high dose in the other trial didn't show a significant benefit. Biogen believed this was because they got the drug for less time. After working with Biogen to analyze the data, the FDA described the results as "exceptionally persuasive." An outside advisory panel disagreed, but none of its members actively treated Alzheimer's patients.

The panel lambasted Biogen for massaging the data. Groups on the left attacked the FDA for collaborating with Biogen. They claimed the FDA would be lowering its standards and compromising its objectivity if it approved the drug. But collaboration between drug makers and FDA is why Americans have three effective Covid-19 vaccines.

As for complaints about insufficient evidence, Congress amended federal law in 1997 to let the FDA approve novel drugs based on a single study in order to accelerate breakthroughs for hard-to-treat diseases. The FDA has appropriately exercised its discretion, and Biogen plans to conduct additional studies following drug approval.

Critics also argued that aducanumab's benefits are modest, but tell that to patients and their caregivers. Incremental progress often drives drug development. Aducanumab won't help all patients, but it could delay disease progression as research continues on potentially more effective treatments. More than 70 are in the clinical pipeline.

The real progressive objection seems to be cost more than effectiveness. New blood tests can identify the disease years before signs of cognitive decline, so the potential patient pool may soon expand. But Alzheimer's costs the U.S. an estimated $277 billion a year, and early treatment will be crucial to slowing progression.

The FDA's approval offers much-needed reassurance to U.S. drug makers that it won't bow to political pressure and let biotech breakthroughs go to waste." [1]
